What is liposuction?

Liposuction, also called liposuction, is a surgical procedure that consists of removing fatty tissue from specific areas of the body, such as the belly, BBL, hips and thighs, using a technique that uses suction.

Once the patient is anesthetized, the procedure can be started. Liposuction is performed with the insertion of a thin cannula, hollow cylindrical metal instrument, through small incisions in the skin towards the layer of adipose tissue (fat tissue). The liposuction cannula is connected to a device that creates a vacuum to suck in fat. When the cannula enters the subcutaneous fat layer, it causes trauma to the cannula, releasing the fat and facilitating its aspiration out of the body.

What I explained above is the basic principle of liposuction. There are currently variations of this technique that allow a more efficient and less traumatic fat aspiration.

Techniques

Infiltrative and non-infiltrative techniques

An infiltrative technique is one in which liposuction is preceded by the injection, in the fat that will be liposuctioned, of a solution composed of serum, vasoconstrictors and, sometimes, local anesthetics.

The injected solution causes the fat cells to "swell", facilitating liposuction, and causes the arteries and veins to contract, reducing surgical bleeding and edema. Due to these properties, the infiltrative technique is preferred by the vast majority of Plastic Surgeons, to the detriment of the non-infiltrative technique, in which, obviously, nothing is injected.

Ultrasonic liposuction

It involves a cannula capable of emitting ultrasonic energy to "melt" the fat, facilitating its removal. The ultrasound technique can be used in conjunction with the infiltrative technique. Although it seems to be more effective in theory, this technique has some disadvantages such as the need for larger incisions, greater risk of internal injuries, higher cost and longer procedure time.

Laser Liposuction

A technique similar to the one described above, only instead of using ultrasound, it uses laser energy to break down fats. Currently, there is no evidence that these two techniques are superior to isolated infiltrative liposuction.

Vibroliposuction

It is another variant, consisting of a cannula that produces rapid movements back and forth, facilitating the breakdown of fat and its aspiration.

Hydrolipo, mini lipo and lipo light

Liposuction is a surgical procedure, preferably qualified for Plastic Surgeons, with previous training in General Surgery (case of all Plastic Surgeons members of the Brazilian Society of Plastic Surgery) and that should only be performed in a surgical center.

The invention of the terms "hidrolipo", "mini lipo", "lipo light" and others has a single purpose: to circumvent the resolution of the Federal Council of Medicine and to allow other professionals to perform liposuction surgeries.

To make matters worse, "hydrolipo", "minilipo" and "lipolight" are sold to the population as being more modern procedures, which do not require hospitalization and that the patient goes from the office straight to work.

In reality, liposuction performed in the office is much lower. In addition to violating a CFM resolution, it does not provide the safety and environment necessary to perform a surgical procedure, the patient is not sedated, feeling, at best, a nuisance throughout the procedure and is often more financially expensive because it has to be done in several stages, since there are limits of toxicity in the use of local anesthetics.

Therefore, the techniques called minilipo, hidrolipo, lipolight are just a form of liposuction in which less fat is removed. Basically, they present the same indications and risks. Who are the best candidates for liposuction?

Liposuction should not aim at weight loss. The best results are obtained by those patients who have kept their weight stable for some time, but have that impertinent localized fat, despite controlling their diet and regular exercise.

The patient should not undergo liposuction first and then decide to change their eating and physical activity habits. The liposuction area does not usually accumulate fat again, which can cause irregularities if the patient comes to put on weight again.

However, not every patient with localized fat is a candidate for liposuction. The degree of sagging skin can be a deterrent. The flaccidity reflects the reduction, or even the loss, of the skin's elasticity. The greater the sagging, the lower the elasticity. The presence of the notorious stretch marks is also a strong indicator of a skin that is poor in elasticity.

To better explain how important elasticity is, I use a moment in life common to almost all women; pregnancy. As the pregnancy progresses, the skin gradually expands to better support the increase in abdominal volume. Immediately after delivery, the volume returns to normal and the skin retracts slowly over the following weeks to adapt again to the abdominal volume of yore. That's where skin elasticity comes in. How many of you know women who after childbirth didn't even look like they had a child? And those with a flabby abdomen? And the ones that besides being flabby are full of stretch marks? What determines the degree of skin retraction is its elasticity, which, similarly to pregnancy, plays the same important role in liposuction.

We know that in the case of liposuction, they act in the retraction of the skin, in addition to the degree of elasticity, the scar contraction. When performing a lipo, fatty tissue is damaged by the cannulas.

This injury, like any injury, heals. It turns out that the entire healing process in our body evolves with a retraction of neighboring tissues. This is also true in liposuction, where the healing of fatty tissue will lead to skin retraction, thanks to the phenomenon of scar contraction.

When there is small or moderate flaccidity, liposuction can be performed, as we can count on the skin's elasticity and scar contraction. When the flaccidity of the skin is moderate to large, isolated lipo should be avoided, under pain of aggravating it. In these cases, the association of skin resection with liposuction should be considered.

Postoperative

The region submitted to liposuction can be very painful and present edema and ecchymosis (purple spots). The plastic surgeon may recommend using a brace to shape the body and reduce edema. The brace is usually used uninterruptedly for about 1 month, and can be removed only at bath time. Subsequently, the brace is used only at night, at bedtime, for another month.

The patient can and should walk on the first postoperative day to reduce the risk of thrombosis in the veins of the lower limbs. Return to work is usually released after 1 week. The deadline for withdrawing points is also usually 7 days. Physical activities only after at least 1 month.

What are the risks of liposuction?

Liposuction is a surgical procedure and must be treated with due seriousness. Complications common to any surgery, such as bleeding and reactions to anesthesia can also occur in liposuction.

Liposuction is very traumatic and causes large edema in the subcutaneous tissue, which means great mobilization of body fluids.
