General description

Liposuction is a surgical procedure that uses a suction technique to remove fat from specific areas of the body, such as the abdomen, hips, thighs, BBL, arms, or neck. Liposuction also shapes (molds) these areas. Liposuction is also called "liposculpture" and "body shaping."

Liposuction is not typically considered a general method or alternative for weight loss. If you are overweight, you are likely to lose more weight with diet and exercise or through bariatric procedures (such as gastric bypass surgery) than with liposuction.

You may be a candidate for liposuction if you have too much body fat in specific places but have a stable body weight.

Why is it done

Liposuction is used to remove fat from areas of the body that have not responded to diet or exercise, such as the following:

  • Abdomen
  • Superior limbs
  • BBL
  • Calves and ankles
  • Thorax and back
  • Hips and thighs
  • Chin and neck
  • Also, liposuction can sometimes be used for breast reduction or gynecomastia treatment.

When you gain weight, fat cells increase in size and volume. In turn, liposuction reduces the number of fat cells in a specific area. The amount of fat removed depends on the appearance of the area and the volume of fat. The resulting changes to the silhouette are generally permanent, as long as your weight remains stable.

After liposuction, the skin adapts to the new contours of the treated areas. If you have good skin tone and elasticity, it will likely keep it looking smooth. However, if your skin is thin and has little elasticity, the skin in the treated areas may appear loose.

Liposuction does not improve cellulite sags or other irregularities in the skin's surface. Similarly, liposuction does not remove stretch marks.

To be a candidate for liposuction, you must be in good health without conditions that can complicate surgery, such as restricted blood flow, coronary artery disease, diabetes, or a weak immune system.

Risks

As with any major surgery, liposuction carries risks such as bleeding and reaction to anesthesia. Possible complications specific to liposuction include:

Abnormalities in the silhouette. Your skin may look bumpy, wavy, or shriveled due to uneven oil removal, poor skin elasticity, and unusual healing. These changes can be permanent. Damage under the skin from the thin tube (cannula) used during liposuction can give the skin a permanent blotchy appearance.

Accumulation of liquids. Temporary fluid collections (seromas) can form under the skin. This fluid may need to be drained with a needle.

Numbness. You may feel temporary or permanent numbness in the affected area. Temporary nerve irritation may also appear.

Infection. Skin infections are rare but likely. A serious skin infection can be life threatening.

Internal puncture. In rare cases, a cannula that penetrates too deeply can pierce an internal organ. This may require an emergency surgical repair.

Fat embolism. Chunks of loose fat can break off, get trapped in a blood vessel, and collect in the lungs or travel to the brain. A fat embolism is a medical emergency.

Kidney and heart problems Changes in fluid levels as fluids are injected and suctioned can cause life-threatening kidney, heart, and lung problems.

Lidocaine Toxicity. Lidocaine is an anesthetic that is often administered with fluids injected during liposuction to help control pain. Although generally safe, in rare circumstances lidocaine toxicity can occur, causing serious heart and central nervous system problems.

The risk of complications increases if the surgeon is working on larger surfaces of the body or doing multiple procedures during the same operation. Talk to your surgeon about how these risks apply to you.

How do you prepare

Food and medicine

Before the procedure, talk with your surgeon about what to expect from the surgery. Your surgeon will review your medical history and ask about any medical conditions you may have and any medications, supplements, or herbs you are taking.

Your surgeon will recommend that you stop taking certain medications, such as blood thinners or NSAIDs (non-steroidal anti-inflammatory drugs), at least three weeks before surgery. You may also need to have certain lab tests before the procedure.

Other precautions

If your procedure requires removing only a small amount of fat, the surgery can be performed in an office setting. If a large amount of fat will be removed (or if you plan to undergo other procedures at the same time), the surgery could take place in a hospital where you would spend the night after. In either case, schedule someone to drive you home and stay with you for at least the first night after the procedure.

What you can expect

Before the procedure

Before the liposuction procedure, the surgeon may mark circles and lines in the areas of your body to be treated. Photographs could also be taken so that before and after images can be compared.

The manner in which the liposuction procedure is performed depends on the specific technique that is used. Your surgeon will select the appropriate technique based on the goals of your treatment, the area of the body to be treated, and if you have had other liposuction procedures in the past.

Tumescent liposuction. This is the most common type of liposuction. The surgeon injects a sterile solution (a mixture of salt water that helps remove fat, an anesthetic [lidocaine] to relieve pain, and a medicine [epinephrine] that causes blood vessels to constrict) into the area to be treated. The liquid mixture causes the affected area to swell and harden.

Then the surgeon makes small cuts in the skin and inserts a thin tube called a cannula under the skin. The cannula connects to a vacuum that sucks fat and fluids from the body. Your body fluid can be replenished through an intravenous (IV) line.

Ultrasound assisted liposuction. This type of liposuction is sometimes used in conjunction with traditional liposuction. During this, the surgeon inserts a metal rod that emits ultrasonic energy under the skin. This breaks the walls of the fat cells and breaks down the fat for easier removal. A new generation of ultrasound-assisted liposuction (UAL), called VASER-assisted liposuction, uses a device that can improve skin contouring and reduce the possibility of injury to the skin.

Laser assisted liposuction. This technique uses high-intensity laser light to break up the fat and thus remove it. During laser-assisted liposuction (LAL), the surgeon inserts a laser fiber through a small incision in the skin and emulsifies the fat deposits. The fat is then removed through a cannula.

Mechanically assisted liposuction. In this type of liposuction a cannula is used that has a fast back and forth motion. This vibration allows the surgeon to remove hard fat more easily and quickly. Sometimes mechanically assisted liposuction (PAL) can cause less pain and swelling and can allow the surgeon to remove fat more precisely. Your surgeon may select this technique if large volumes of fat need to be removed or if you have had a previous liposuction procedure.

During the procedure

Some liposuction procedures may require only local or regional anesthesia (anesthesia that is limited to a specific area of the body). Other procedures may require general anesthesia, which induces a temporary state of unconsciousness. You may be given a sedative, usually through an IV injection, to help you stay calm and relaxed.

The surgical team will monitor your heart rate, blood pressure, and blood oxygen level throughout the procedure. If you are given local anesthesia and you feel pain during the procedure, inform your surgeon. Medication or movement adjustments may need to be made.

The procedure can take several hours, depending on the amount of fat that is removed.

If you have been given general anesthesia, you will wake up in a recovery room. Typically, you will spend at least a few hours in the hospital or clinic so the medical staff can monitor your recovery. If you are in a hospital, you can spend the night there to make sure you are not dehydrated or in shock from the loss of fluids.

After the procedure

You may have pain, swelling, and bruising after the procedure. Your surgeon may prescribe medications to help control pain and antibiotics to reduce the risk of infection.

After the procedure, the surgeon may leave the incisions open and place temporary drains to promote fluid drainage. Generally, you will need to wear tight compression garments, which help reduce swelling, for a few weeks.

You may have to wait a few days to return to work and a few weeks to resume your normal activities, including physical exercise.

During this time, you can expect some abnormalities in your silhouette, as the remaining fat must accommodate.

Results

After liposuction, the swelling usually clears up within a few weeks. By then, the treated area should look less bulky. After a few months, the treated area should appear slimmer.

It is natural for skin to lose firmness with aging, but the results of liposuction are generally long-lasting as long as you maintain your weight. If you gain weight after liposuction, the distribution of fat can change. For example, you could accumulate fat around the abdomen, regardless of the areas that were originally treated.
